Welcome to Oak Forest Dental! It’s our pleasure to provide your dental care needs. A great relationship with you is our number one goal. 

For us, it’s about more than just your teeth. It’s about getting to know you as a person. The medical history form you may fill out provides information that helps us begin to know you, but we’re eager to learn more about you—what you do for a living, your family, your interests, or what makes you tick.

We know you’ll feel comfortable with the warm and friendly atmosphere that has become our trademark style. Expect to hear laughter at Oak Forest Dental. It’s just how we roll!

Typically, your first visit will include x-ray imaging (if necessary), an initial cleaning of your teeth, and a full examination of your mouth. If we have treatment recommendations, we’ll explain the reasons why, so you can make well-informed decisions about your dental care. We love to answer your questions because we believe that good dental health is based on good understanding of your conditions.

Please download and complete our new patient forms, available using the link below. Once completed, you may email them to [email protected] , or print them and bring them along with you to your appointment.

If you are not able to complete these forms prior to your appointment, please plan to arrive approximately 15–30 minutes before your scheduled appointment time so you may fill them out at our office.
